Details for all full-time Gifted Healthcare employees.
If you regularly work 30 or more hours per week, you are eligible for benefits. You won’t be able to change your benefits until the next enrollment period unless you experience a Qualifying Life Event. If a Qualifying Life Event occurs, you have 30 days to request changes to your coverage.
Your Basic Life and AD&D insurance benefit is $50,000. If you are a full-time employee, you automatically receive Life and AD&D insurance even if you elect to waive other coverage.

Basic (No Cost)
Basic Life and AD&D benefits are provided to you as a part of your basic coverage. Gifted Healthcare provides employees with Basic Life and AD&D insurance through Lincoln Financial Group, which guarantees that loved ones, such as a spouse or other designated survivor(s), continue to receive part of an employee’s benefits after a death.
You may purchase Voluntary Life insurance for yourself in increments of $10,000 or 5x annual earnings, up to a maximum of $500,000. Guarantee Issue is $100,000. If you previously waived coverage and are electing for the first time, Evidence of Insurability (EOI) will be required.
Premium deductions for Voluntary Life/AD&D are taken from your paycheck after taxes. Rates are available online during enrollment.
